How is Xanax XR different from regular Xanax?

The difference is the speed of release, not the warning. Xanax XR is absorbed slowly and taken once a day, while the standard tablet peaks within a couple of hours and is taken more often. The FDA label applies the same withdrawal warning to both.

A slower release curve is not the same as a gentler withdrawal. The label reports no such difference, and its seizure warning covers both forms. Anyone moved from one form to the other, or thinking about a switch, should settle it with the prescriber. Tell admissions which form is involved, because it changes how the timing of doses is read.

Between doses

Why do some people feel anxious between Xanax doses?

The FDA label reports early-morning anxiety and anxiety between doses in people taking prescribed maintenance doses of Xanax for panic disorder. It offers two possible reasons: tolerance has developed, or the gap between doses is longer than each dose keeps working.

The label describes it in panic-disorder patients on a prescribed schedule. It does not say how often it happens in people taking alprazolam for generalized anxiety, or in anyone taking it without a prescription.

For a family, the useful conclusion is what not to do. Anxiety that returns before the next dose is a reason to call the prescriber, not to take an extra tablet early and not to quit. If you are seeing this pattern:

  1. Note the times: when each dose is taken and when the anxiety returns.
  2. Bring those notes to the prescriber, or to admissions if the pills are not prescribed.
  3. If the Xanax is prescribed, leave the dose and schedule as they are until the prescriber has looked at them. If it is not, call admissions today: a sudden stop can still bring withdrawal, and street pills carry their own danger.

Telling them apart

Xanax withdrawal symptoms: withdrawal, rebound or the anxiety returning?

Some Xanax withdrawal symptoms can be identified as withdrawal; anxiety and insomnia often cannot. In a controlled trial of 63 patients described on the FDA label, researchers sorted what appeared while alprazolam was stopped into the groups below.

Why the overlap matters. Anxiety and broken sleep are the symptoms families notice first, and they are exactly the ones the trial could not explain. The label adds that, for panic disorder, stopping after a long stretch without attacks can be hard without symptoms returning or withdrawal appearing. A clinician watching over several days is in a far better position to sort them than anyone guessing at home. Short use

Can you become dependent on Xanax after a short time?

Some risk exists even after relatively short-term use, according to the FDA label. Spontaneous reports suggest the risk and severity of withdrawal are greater at higher doses and with use beyond about 12 weeks.

The label identifies people on higher doses and longer courses as the ones most at risk of withdrawal. In one study it describes, dose mattered more than duration: among people treated for panic disorder, three months versus six months of treatment did not change how well they could come off alprazolam, while higher doses made it harder.

Short use still counts. A few weeks of prescribed Xanax is not proof that stopping will be uneventful. It is a reason to plan the stop with the prescriber instead of quitting abruptly or simply running out.

Counterfeit pills

Fake Xanax and bromazolam: is the pill really alprazolam?

A pill that did not come from a pharmacy may not be alprazolam at all. The DEA says many fake pills are made to look like Xanax and other prescription drugs but contain fentanyl or methamphetamine, and the CDC has found illicit benzodiazepines such as bromazolam in pills sold as alprazolam.

What do CDC overdose records show?

In 2021, the CDC found evidence of counterfeit pills in 2,437 of 54,768 overdose deaths (4.4%) across 35 jurisdictions. The share grew fastest in western jurisdictions, rising from 4.7% to 14.7% of deaths between mid-2019 and the end of 2021, while other regions stayed below 4%. Of those 2,437 deaths, 415 involved counterfeit alprazolam alone and 95 involved counterfeit alprazolam with oxycodone. Nearly one in five counterfeit-alprazolam deaths also involved illicit benzodiazepines, and illicitly made fentanyl was present in 93% of all deaths with counterfeit-pill evidence.

What happened when bromazolam was sold as alprazolam?

In February 2023, three young adults in Chicago took pressed tablets they believed were alprazolam. Testing found bromazolam, a designer benzodiazepine never approved for medical use, and no fentanyl or other opioid. Naloxone brought no response. All three had seizures and injury to the heart muscle, and one still had speech and swallowing problems at discharge. Illinois recorded 10 bromazolam-involved deaths in 2021 and 51 in 2022. It is one case series of three people, not a rate, but it shows what an unregulated pill can hide.

Why does a fake pill change detox?

Stopping

Why should Xanax never be stopped all at once?

A sudden stop or a fast cut is the step most likely to bring on severe withdrawal, and the label names a seizure as its most important event. MedlinePlus advises against stopping or reducing alprazolam without the doctor, who will likely lower the dose gradually.

That gradual plan is the prescriber’s to set, for one person at a time. Nothing on this page is a schedule, and no website can supply one.

Why does mixing raise the stakes?

The label notes that deaths linked to alprazolam misuse more often involve other substances too, especially opioids and alcohol. Call 911 if someone has collapsed, had a seizure, is struggling to breathe or cannot be woken, whether the pill was prescribed or not. Can you die from Xanax withdrawal?

It can be life threatening. The FDA label names seizures as the most important withdrawal event, and the risk is highest after a sudden stop or a fast cut, at higher doses and after longer use. A seizure during withdrawal is a reason to call 911 straight away.

Why do I feel anxious before my next Xanax dose?

Is the anxiety during Xanax withdrawal just my anxiety coming back?

At first it may be impossible to say. In a controlled trial described on the label, anxiety and insomnia were common while alprazolam was being stopped, but researchers could not tell whether they came from the original illness, rebound or withdrawal. Muscle twitching, blurred vision and other symptoms were judged to be withdrawal.

Will naloxone help if someone took a fake Xanax?

Give it if you have it, and call 911. Fake Xanax can contain fentanyl, and naloxone reverses opioid overdose. SAMHSA notes that it does not treat a benzodiazepine overdose, and in one CDC case report it produced no response in people who had taken bromazolam sold as alprazolam, so emergency care is still essential.

Is it dangerous to take Xanax that was prescribed to someone else?

Yes. MedlinePlus says alprazolam should be taken exactly as directed and never shared, and the FDA label counts taking another person’s prescription as misuse. A pill passed along by a friend may not have come from a pharmacy at all, which adds the counterfeit risk described on this page.

What should I tell admissions if the Xanax didn’t come from a pharmacy?

Where it came from, what it looked like, how often it was taken and what else is used, including alcohol and opioids. Nobody can know what a street pill held, so the assessment has to allow for a body that may be dependent on something other than alprazolam.
